Terms of Use & Disclosures

These Terms of Use & Disclosures (the “Terms”) govern your access to and use of seniorreliancegroup.com, any related webpages, forms, questionnaires, quizzes, telephone numbers, text-message programs, email communications, and other online features operated by Senior Reliance Group LLC (collectively, the “Site”).
In these Terms, “Senior Reliance Group,” “SRG,” “we,” “us,” and “our” mean Senior Reliance Group LLC. “You” and “your” mean the individual accessing or using the Site. “Partner” means an independent third-party professional or business within, or introduced through, our contracted referral network.
1. Acceptance of These Terms
By accessing or using the Site, submitting information, or otherwise interacting with us through the Site, you acknowledge that you have read, understood, and agree to these Terms and our separate Privacy Policy.
If you do not agree, do not use the Site or submit information through it.
You must be at least 18 years old and legally capable of entering into a binding agreement to use the Site. If you use the Site on behalf of another person, you represent that you are legally authorized to provide the information and request assistance for that person.
2. Who We Are
Senior Reliance Group LLC is a marketing, information, matching, and referral company. We provide a helping hand by collecting limited information about what a consumer is seeking and attempting to connect that consumer with one or more independent Partners who may offer relevant products, services, information, or assistance.
SRG is not the company that ultimately provides, sells, performs, administers, underwrites, delivers, or guarantees any Partner product or service unless we expressly state otherwise in a separate written agreement signed by SRG.
Our referral and matching activities may include areas such as:
- diabetes-related solutions, supplies, resources, or support;
- insurance information and access to independent licensed insurance agents; and
- home-care or in-home support services.
Available categories, Partners, products, and services may change and may not be available in every location.
3. We Are Not a Service Provider
Senior Reliance Group is not a healthcare provider, physician, medical practice, pharmacy, durable medical equipment supplier, home-care agency, home-health agency, insurance company, insurance carrier, government agency, law firm, accounting firm, financial institution, or emergency service.
We do not diagnose, treat, prescribe, furnish medical equipment, provide hands-on care, issue insurance coverage, determine insurance eligibility, approve claims, establish plan benefits, supervise caregivers, or perform the services offered by Partners.
Use of the Site does not create a doctor-patient, caregiver-client, insurer-insured, agent-client, fiduciary, legal, financial, or other professional relationship between you and SRG.
4. Independent Partners; No Agency Relationship
Every Partner is an independent third party. Partners are not employees, agents, representatives, joint venturers, franchisees, or legal partners of SRG. SRG does not direct or control a Partner’s professional judgment, staffing, scheduling, pricing, sales practices, eligibility decisions, underwriting, service methods, contract terms, performance, or customer relationships.
Although we may use internal business criteria when deciding which Partners to include or contact, a referral or introduction does not constitute a certification, endorsement, warranty, or guarantee of any Partner or of any Partner’s licensing, qualifications, background, availability, suitability, products, services, advice, conduct, or results.
You are responsible for independently evaluating a Partner before sharing additional information, signing documents, paying money, accepting services, or making any health, insurance, financial, or care-related decision. You should confirm any licenses, credentials, insurance, references, pricing, scope, exclusions, and contract terms that are important to you.
5. SRG Is Not a Party to Your Agreement With a Partner
Any quotation, application, enrollment, order, prescription, authorization, plan, policy, service agreement, payment arrangement, care plan, product purchase, or other transaction entered into with a Partner is solely between you and that Partner.
SRG is not a party to, beneficiary of, guarantor of, or obligor under any agreement between you and a Partner. We do not assume a Partner’s duties, and a Partner cannot create obligations or liability on behalf of SRG.
Any concern, complaint, refund request, billing issue, service dispute, injury claim, coverage dispute, product issue, cancellation, or other claim arising from a Partner’s products, services, representations, employees, contractors, or agreement must be addressed directly with that Partner and, when appropriate, the relevant licensing authority, regulator, insurer, or other responsible entity.
You may not identify or treat SRG as the provider of a Partner’s product or service merely because SRG introduced you to the Partner or transmitted information at your request.
6. No Government Affiliation
Senior Reliance Group is not affiliated with, endorsed by, authorized by, or connected to the United States government, any state or local government, Medicare, Medicaid, the Centers for Medicare & Medicaid Services (CMS), the Social Security Administration, or any other federal or state agency or public-benefit program.
The Site is not a government website. Any government program names used on the Site are for identification and educational purposes only.
7. No Professional Advice
Information provided through the Site is general information only. SRG does not provide medical, healthcare, legal, insurance, tax, investment, or financial advice. Nothing on the Site should be treated as a diagnosis, treatment recommendation, coverage determination, legal opinion, or substitute for advice from an appropriately licensed professional who has evaluated your circumstances.
Do not disregard or delay professional medical advice because of information found on the Site. For a medical emergency, call 911 or your local emergency number. SRG does not provide emergency assistance.
8. Insurance and Medicare-Related Disclosures
SRG does not issue insurance policies, determine eligibility, underwrite risk, set premiums, approve applications, or decide claims. Insurance information and recommendations, if any, are provided by independent licensed insurance professionals and are subject to carrier rules, policy terms, eligibility requirements, availability, and applicable law.
Submission of information through the Site is not an insurance application, does not create coverage, and does not guarantee that a particular plan, policy, premium, benefit, product, or provider will be available or appropriate.
Senior Reliance Group does not market, sell, endorse, or provide Medicare Advantage plans, Medicare Prescription Drug (Part D) plans, or other Medicare-regulated products, and is not a Medicare marketing organization.
9. Diabetes and Health-Related Information
SRG does not determine medical necessity, prescribe or furnish diabetes supplies, verify insurance benefits, make coverage decisions, or guarantee that a healthcare item or service will be ordered, covered, reimbursed, or delivered.
Any healthcare evaluation, prescription, order, fulfillment, coverage verification, or treatment relationship is handled solely by the applicable licensed professional, supplier, provider, pharmacy, payer, or other responsible third party.
10. Home-Care Services
SRG does not employ, schedule, supervise, train, credential, or control a Partner’s caregivers or home-care personnel unless expressly stated in a separate written agreement. The Partner is solely responsible for its personnel, licensing, background checks, insurance, care plans, service quality, scheduling, safety practices, and compliance with law.
You are responsible for reviewing the Partner’s qualifications, service agreement, staffing practices, rates, cancellation terms, and safeguards before accepting services.
11. Information You Provide
We request limited information reasonably related to identifying and facilitating a potential referral, such as your name, phone number, email address, ZIP code, general location, preferred contact method, and the category of assistance requested.
You agree to provide information that is accurate, current, and lawfully provided. If you submit information about another person, you represent that you have appropriate authority and permission to do so.
Do not submit sensitive or protected information through a general Site form, including:
- Social Security numbers;
- bank-account, routing, debit-card, or credit-card numbers;
- account passwords or security codes;
- detailed medical records or diagnostic reports;
- insurance-member identification numbers;
- government-issued identification numbers; or
- any other information the form does not specifically request.
SRG does not request or intend to collect this information through general inquiry or matching forms. If a Partner requires additional information, the Partner must collect it through its own secure and legally compliant process.
Our collection, use, disclosure, and retention of information are also governed by our separate Privacy Policy, which is incorporated into these Terms by reference.
12. Permission to Use and Share Submitted Information
When you submit information and request a referral, you authorize SRG to use the information to:
- review and process your request;
- identify potentially relevant Partners;
- transmit the information you submitted to selected Partners for the requested purpose;
- contact you about your request;
- operate, protect, and improve the Site and referral process; and
- comply with legal and regulatory obligations.
SRG will not represent that a Partner has accepted your request until that Partner actually does so. Sharing information does not guarantee a response, qualification, approval, coverage, appointment, or service.
13. Consent to Calls, Texts, and Emails
Your use of the Site does not, by itself, require you to consent to marketing calls or texts. Where consent is legally required, we will request it through a separate, clear disclosure presented with the relevant form or checkbox.
If you affirmatively provide the requested consent, you authorize Senior Reliance Group and the specifically identified or selected Partners disclosed at the point of consent to contact you at the telephone number and email address you provide about the products or services you requested. Contact may occur by telephone call, text message, or email and may include communications made using an automatic telephone dialing system, prerecorded or artificial voice, or other automated technology, if clearly stated in the consent you accepted.
Consent is not a condition of purchasing any property, goods, or services. Message and data rates may apply. Message frequency may vary.
You may revoke consent at any time by any reasonable method that clearly communicates your request. You may reply STOP to a text message, ask a caller not to contact you again, use any opt-out mechanism provided, or contact us at legal@seniorreliancegroup.com or (940) 629-2836. For text assistance, reply HELP when that option is available.
SRG and its Partners must maintain and honor their own legally required suppression and Do-Not-Call procedures. An opt-out sent to SRG may not automatically reach an independent Partner unless the law requires it or we expressly confirm otherwise; therefore, you should also send an opt-out directly to any Partner contacting you.
Important implementation requirement: The operative call/text consent must appear conspicuously at the point of collection and should not be hidden solely inside these Terms. Counsel must approve the exact checkbox language, Partner identification method, consent records, revocation workflow, calling technology, call scripts, hours, suppression procedures, and state-specific requirements before launch.
14. Partner Network and Compensation
Partners are independent businesses that may contract with SRG for marketing, referral, administrative, or related services. SRG may receive a fee or other compensation when we provide a referral, when a Partner contacts a consumer, when a consumer completes a qualifying action, or when a transaction occurs.
Compensation may affect which Partners are presented or contacted. SRG does not promise to identify every potentially available professional, provider, product, plan, or service in your area, and our network may not include all available options.
You are not charged by SRG for using the Site unless a separate written disclosure expressly states otherwise. A Partner may charge for its own products or services under its separate agreement with you.
15. No Guarantee
SRG does not guarantee:
- that a Partner will contact or accept you;
- that any product or service will be available in your area;
- that you will qualify for insurance, benefits, coverage, supplies, financing, care, or assistance;
- that a Partner’s information, recommendations, pricing, availability, or services will be accurate, complete, timely, suitable, or satisfactory;
- that any referral will result in savings, approval, improved health, coverage, reimbursement, or a particular outcome; or
- that the Site will always be available, uninterrupted, secure, or error-free.
16. Your Responsibilities
You agree to:
- use the Site only for lawful, personal purposes;
- provide truthful and accurate information;
- obtain permission before submitting another person’s information;
- independently evaluate Partners and their offerings;
- read all Partner agreements, disclosures, exclusions, authorizations, and notices before agreeing to them;
- ask questions and seek independent professional advice when appropriate; and
- comply with applicable laws and these Terms.
You may not interfere with the Site, attempt unauthorized access, submit malicious code, impersonate another person, scrape or copy the Site at scale, use the Site to harass others, or use information obtained through the Site for unlawful or unauthorized purposes.
17. Third-Party Websites and Materials
The Site may link to or display information from Partners or other third parties. SRG does not control third-party websites, applications, privacy practices, security, content, products, or services. A link or referral does not mean that SRG adopts or guarantees the third party’s statements or conduct.
Your use of a third party’s website or services is governed by that third party’s own terms, privacy policy, disclosures, and agreements.
18. Intellectual Property
The Site and its original content, branding, logos, graphics, layout, text, software, and other materials are owned by or licensed to SRG and are protected by applicable intellectual-property laws.
SRG grants you a limited, revocable, nonexclusive, nontransferable license to access and use the Site for its intended personal purpose. No other rights are granted. You may not reproduce, modify, sell, publish, distribute, reverse engineer, create derivative works from, or commercially exploit Site materials without prior written permission.
19. Disclaimer of Warranties
TO THE FULLEST EXTENT PERMITTED BY LAW, THE SITE, REFERRAL PROCESS, INFORMATION, COMMUNICATIONS, AND ANY INTRODUCTIONS ARE PROVIDED “AS IS” AND “AS AVAILABLE,” WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED.
SRG disclaims all warranties of merchantability, fitness for a particular purpose, title, noninfringement, accuracy, completeness, availability, security, reliability, and any warranties arising from course of dealing or usage of trade.
SRG makes no warranty regarding any Partner or any Partner product, service, advice, personnel, licensing, conduct, agreement, or result.
Some jurisdictions do not allow certain warranty exclusions, so portions of this section may not apply to you.
20. Release Concerning Partner Dealings
To the fullest extent permitted by law, you acknowledge that your dealings with a Partner are at your own discretion and risk, and you release and hold harmless SRG and its owners, officers, directors, employees, contractors, and affiliates from claims, demands, losses, liabilities, and disputes arising solely from:
- a Partner’s acts, omissions, statements, advice, products, services, personnel, billing, privacy practices, or contract;
- your decision to select, reject, hire, enroll with, purchase from, or otherwise deal with a Partner; or
- a disagreement between you and a Partner.
This release does not waive liability that cannot lawfully be waived and does not release SRG from claims based directly on SRG’s own fraud, willful misconduct, gross negligence where it cannot be disclaimed, or other nonwaivable legal duties.
21. Limitation of Liability
TO THE FULLEST EXTENT PERMITTED BY LAW, SRG AND ITS OWNERS, OFFICERS, DIRECTORS, EMPLOYEES, CONTRACTORS, AND AFFILIATES WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, EXEMPLARY, CONSEQUENTIAL, OR PUNITIVE DAMAGES; LOSS OF PROFITS, REVENUE, DATA, GOODWILL, OR OPPORTUNITY; PERSONAL OR BUSINESS INTERRUPTION; OR COST OF SUBSTITUTE PRODUCTS OR SERVICES ARISING OUT OF OR RELATED TO THE SITE, A REFERRAL, A PARTNER, OR THESE TERMS, EVEN IF ADVISED THAT SUCH DAMAGES MAY OCCUR.
To the fullest extent permitted by law, SRG’s total aggregate liability for all claims arising out of or relating to the Site, referral process, or these Terms will not exceed the greater of:
- the amount, if any, you paid directly to SRG for use of the Site during the twelve months before the event giving rise to the claim; or
- one hundred U.S. dollars ($100).
SRG is not liable for the acts or omissions of an independent Partner and is not responsible for fulfilling, refunding, correcting, defending, or paying obligations arising under your agreement with a Partner.
These limitations do not apply to liability that cannot be limited under applicable law. Because some jurisdictions restrict exclusions or limitations of damages, some provisions may not apply to you.
22. Indemnification
To the fullest extent permitted by law, you agree to defend, indemnify, and hold harmless SRG and its owners, officers, directors, employees, contractors, and affiliates from third-party claims, damages, losses, judgments, liabilities, penalties, costs, and reasonable attorneys’ fees arising from:
- your unlawful or unauthorized use of the Site;
- information you submit without authority or permission;
- your material violation of these Terms;
- your infringement of another person’s rights; or
- fraud, misconduct, or unlawful acts committed by you through or in connection with the Site.
This section does not require you to indemnify SRG for SRG’s own conduct to the extent such indemnification is prohibited by law.
23. Changes to the Site or These Terms
We may modify the Site and these Terms from time to time. The “Last updated” date will identify the most recent revision. Material changes will be communicated as required by applicable law.
Your continued use of the Site after revised Terms become effective constitutes acceptance of the revised Terms, except where applicable law requires additional notice or affirmative consent.
24. Suspension or Termination
We may restrict, suspend, or terminate access to the Site if we reasonably believe a user has violated these Terms, created legal or security risk, misused the Site, or engaged in unlawful conduct. Provisions that by their nature should survive termination will survive, including provisions addressing Partner independence, disclaimers, releases, limitations of liability, indemnification, and dispute resolution.
25. Governing Law and Dispute Resolution
These Terms are governed by the laws of the State of Wyoming, without regard to conflict-of-law principles, except to the extent federal law applies.
Before filing a claim, you and SRG agree to make a good-faith effort to resolve the dispute informally by providing written notice describing the issue and requested resolution. Notices to SRG should be sent to legal@seniorreliancegroup.com and to 30 N Gould St, Ste N, Sheridan, WY 82801.
Any lawsuit that is not resolved informally must be brought in the state or federal courts located in Sheridan County, Wyoming, unless applicable law requires another forum.
26. General Provisions
These Terms and the Privacy Policy constitute the entire agreement between you and SRG concerning the Site unless a separate written agreement expressly applies.
If any provision is found unenforceable, it will be modified to the minimum extent necessary or severed, and the remaining provisions will remain effective. SRG’s failure to enforce a provision is not a waiver. You may not assign your rights under these Terms without our written consent. SRG may assign these Terms in connection with a merger, acquisition, reorganization, sale of assets, or operation of the Site.
Headings are for convenience only and do not limit the meaning of any provision.
